Before you submit any personal information to Ayogo Health USA Inc. (“Company”) please read this Privacy Policy carefully.  By accessing Company’s Application or website (the “Site”), or using any of Company’s services, you are accepting the policies and practices described in this Privacy Policy.

Personal Information Collected

Company receives and stores any information you enter on the Site, through the Application, or provide to Company in any other way.  This includes information that can identify you (“Personal Information”), including your first and last name, telephone number, postal and e-mail addresses, user name and password.  It may also include individually identifiable health information (see Section 2 below).  The Site or Application may also request or you may provide to the Site or Application information about your personal circumstances, language and location preferences.  Additional information may be required in order to initiate other transactions or activities on the Site or through the Application.  You may also have the opportunity to provide contact information for other people through the Site. HIPAA Authorization and Consent

Company receives and stores any personal health information you enter on the Site, through the Application or provide to the Company in any other way.  Your personal health information is kept confidential except as is necessary to provide you with the services offered by the Site or Application, and is subject to the same terms of collection, use and disclosure as other personal information you provide to Company, as per the terms of this Policy.  Each time you use the Application or access the Site, or use any of Company’s services, you consent and agree to the collection, use and disclosure of your personal health information as per the terms of this Privacy Policy.

Other Information Collected

Company may automatically collect some information about your computer when you visit the Site, including session data, IP address, and Web browser software.  Company may also collect information about your online activity, such as content viewed and pages visited, the frequency of your postings in the Forums, or your progress in any game.

Company will not share your email address with any other members nor display it any public manner, except where you have given Company express permission to do so.  Company may share your email address with selected service partners, except where you have expressly requested that Company not do so.

Other than as set out above, you will be notified when personal information about you will be shared with third parties, and you will have an opportunity to choose not to have us share such information.

Company may update this Privacy Policy in the future.  We will notify you of material changes to this Privacy Policy be either sending a notice to the email address you provided to us, or by placing a notice on the Site.
